Overall Services

We support and assist individuals with mental and physical disabilities in all aspect of their lives.

CONTINUOUS RESIDENTIAL HABILITATION

  • Services are provided in a provider-controlled home with 24/7 staff support. 
  • The provider works with individuals to develop independent living skills and participate in community activities. 
  • Support also includes assistance with daily living tasks, health maintenance, social engagement, leisure activities, and ongoing supervision. 

COMMUNITY INTEGRATION (CI)

  •  Our staff and caregivers provide individualized support tailored to each person’s needs, skills, abilities, and interests. 
  • Our programs are designed to offer meaningful experiences and engaging activities that promote physical well-being, cognitive growth, and social connection. 
  • Activities include trips to museums, libraries, zoos, parks, and malls, as well as visits to the Humane Society, bowling outings, and community volunteering opportunities 

DAY SUPPORT

TEAM-LLC offers individuals the ability to learn about arts at  its day location.

SHARED LIVING PROVIDER (SLP)

 

 

What is a Shared Living Provider (SLP)?

A Shared Living Provider (SLP) is an individual or family who opens their home to a person with developmental or intellectual disabilities, providing them with personalized care and support in a safe, family-like environment. Unlike traditional facility-based care, shared living focuses on integration, independence, and inclusion, helping each individual live a meaningful life within a community setting.

Key Responsibilities of a Shared Living Provider

  1. Daily Care & Support
     
    • Assists with Activities of Daily Living (ADLs) such as bathing, grooming, dressing, meal preparation, and mobility support.
       
    • Provides supervision and ensures the individual’s safety at home and in the community.
       

  1. Health & Well-Being
     
    • Monitors and supports the individual’s medical needs, including medication administration and attending medical appointments.
       
    • Promotes healthy living habits (nutrition, hygiene, exercise, and rest).
       

  1. Skill Development
     
    • Teaches and reinforces independent living skills such as cooking, cleaning, budgeting, and personal care.
       
    • Encourages participation in community activities like shopping, volunteering, and recreational events.
       

  1. Social & Emotional Support
     
    • Provides companionship and emotional stability in a nurturing environment.
       
    • Encourages the development of friendships, hobbies, and social networks.
       

  1. Advocacy & Inclusion
     
    • Advocates for the individual’s rights, dignity, and opportunities.
       
    • Ensures they are included in community life, education, employment, and recreational opportunities.

FOSTER CARE TRAINING & PLACEMENT

 

At TEAM-LLC (Together Everyone Achieves More, LLC), we believe that every child deserves a safe, nurturing, and supportive home where they can grow, heal, and thrive. Through our Foster Care Training and Placement Program, we work hand in hand with families and communities to ensure foster children receive the love, stability, and guidance they need.

Foster Care Placement

TEAM-LLC is licensed to provide foster care placements for children who cannot safely remain in their biological homes. Our placement process is carefully structured to match each child with the right foster family:

  • Safe and Loving Homes: Children are placed in family environments where they are valued, respected, and cared for.
     
  • Individualized Matching: Placements are made based on the child’s age, needs, interests, and unique circumstances.
     
  • Ongoing Case Management: Our team works closely with foster parents, biological families (when appropriate), and caseworkers to ensure the best outcomes for children.
     
  • 24/7 Support: Foster families receive around-the-clock support to navigate challenges and celebrate successes.
     

Foster Care Training

TEAM-LLC prepares foster families through comprehensive training programs designed to equip them with the skills and confidence needed to care for foster children:

  • Pre-Service Training: Foster parents complete certification training that covers trauma-informed care, child development, safety, and legal requirements.
     
  • Ongoing Education: Continuous learning opportunities, including workshops, online modules, and in-person coaching.
     
  • Specialized Topics: Training includes behavior management, cultural competency, emotional regulation, attachment and bonding, and supporting children with special needs.
     
  • Peer and Professional Support: Foster parents are connected to mentors, peer networks, and professional staff for ongoing encouragement and advice.

DAY & EVENING REPORTING CENTER FOR FOSTER CHILDREN

 

we understand that foster children often face unique challenges, emotional, academic, and behavioral that require specialized, consistent support beyond the home and school environment. Our Day and Evening Reporting Center (DERC) is designed to provide foster youth with a safe, structured, and nurturing space that promotes stability, growth, and positive outcomes.

Program Purpose

The DERC program serves as both a preventive and supportive intervention, giving foster children a constructive environment where they receive guidance, supervision, and opportunities to learn and grow. The center operates during daytime and evening hours, ensuring flexibility for foster families, caseworkers, and the courts.

Services Provided

1. Academic Support

  • Tutoring and homework assistance.
     
  • Collaboration with schools to ensure educational progress.
     
  • Study skills and literacy programs.
     

2. Life Skills Development

  • Training in Activities of Daily Living (ADLs).
     
  • Financial literacy and budgeting basics.

3. Behavioral and Emotional Support

  • Trauma-informed interventions.
